Upload Instructions

To upload your COVID-19 vaccination documentation, students and employees must coordinate on Self-Service. First-year students must complete our Immunization Form or may request an exemption if necessary.

Student Uploads 

  • Log in to Self-Service
  • Select the ‘Student’ tab
  • Click ‘Upload Document Files’
  • Department Request: COVID DOCUMENTS
  • Document Type: COVID Immun Rec
  • Click ‘Choose File’ to upload a picture of your card
  • Then ‘Submit’ to complete


Employee Uploads 

  • Log in to Self-Service
  • Select the ‘Employee’ tab
  • Click ‘File Upload’
  • Department Request: HR
  • Document Type: COVID Immun Rec
  • Click ‘Choose File’ to upload a picture of your card
  • Then ‘Submit’ to complete


Exemption Submissions 

Students and employees who cannot receive the vaccine for medical reasons and sincerely held religious beliefs will also be included in the giveaway once their exemption has been approved.

To submit an exemption form, students should email immunizationrecords@mountsaintvincent.edu and employees should email hrcovidexemption@mountsaintvincent.edu.
