4.2 Faculty Personnel File
The faculty member’s file includes:
- All pre-employment information;
- Copies of all course evaluations;
- Classroom observations and performance appraisal reports;
- Letters;
- Course evaluations; and,
- Observation reports conducted by the Excellence in Teaching Committee, which are voluntarily submitted by the faculty member; and records of any appeals.
The Faculty file is maintained in the Office of the Provost/Dean of Faculty. Department Chairs are expected to maintain faculty personnel files in the Department. All personnel files are confidential and may be consulted only by the individual faculty member and those who have supervisory authority over the individual faculty. Files must not be removed from the Office of the Provost/Dean of Faculty or departmental office.
